Application for Disability Support Services
A student who wishes to receive disability-related accommodations must register with the Disability Resource Center (DRC). To begin the registration process, (1) complete the following application and (2) submit documentation of a disability. Disability documentation should appear on official letterhead and be signed by a professional who practices in a field or specialty directly related to the stated disability.
DRC will review the application and documentation. If all is in order, DRC will contact you through your MSU email address to make an appointment.
